Each presenter must submit these 3 forms prior to attending the Emergency Medicine in Rural Hospitals Conference.

Completing these forms is a necessary step for confirming your participation as a speaker in the EMRH 2026 conference.

These forms must be submitted by Monday October 6, 2025 at 11:59PM.

Speaker Content Waiver Form

By completing this form, I am confirming my understanding and agreement regarding how my course content will be used by the CME & PD Office.

Handouts: The materials I provide (slides, notes, or other handouts) may be printed and distributed to course registrants.
Presentation Recording: My presentation and my image/voice may be recorded in podcast, audio, video, or still image format.
Access for Registrants: Both my handouts and presentation recordings may be posted on a password-protected site managed by the CME & PD Office (www.ecme.ucalgary.ca) for the exclusive use of registrants, for up to 12 months following the program date.
Copyright Compliance: I will ensure that all materials I provide comply with the University of Calgary Copyright Policy. For guidance, I understand that I can contact the Copyright Office at copyright@ucalgary.ca.
Consent to Participate: I consent to take part in the identified program or project and understand that my information will only be used for the purposes listed above.
