This approach leverages data from your practice to drive targeted improvements that are directly relevant to your daily work and patient outcomes.

When working on a practice-driven quality improvement (QI) project, identifying and gathering relevant data is crucial to understanding your practice’s current performance and recognizing areas for improvement. Several options are available to you for sourcing your personalized practice data. This course supports the generation of data through the SUPPORT for PPIP: Self Reflection Tool for Data-informed QI.

How can the SUPPORT for PPIP Tool for Data-informed QI generate data for PPIP's Practice-driven QI activity?

The data generation tool in the SUPPORT for PPIP requires you to review a minimum of 10 patient charts  and answer questions based on the CPSA’s Standards of Practice. You will not be asked to input any identifiable patient information. As you go through your records, ensure that you are consistently reflecting on the information and using it to drive meaningful changes in your practice.

To fulfill the requirements for both quality improvement activities in the PPIP course, you need to complete two distinct quality improvement (QI) projects. This means that the Standards of Practice QI activity and the Practice Driven QI activity each need independent data generation and an independent action plan. However, generating data for these two activities can follow the same process: through a SOP questionnaire.

You must select 2 Different Standards of Practice (SOPs) questionnaires

  • One for the Practice-Driven QI Activity, and
  • One for the CPSA Standards of Practice QI Activity.
    •  
    • This ensures that you address varied aspects of your practice and meet the requirements of the PPIP program for the 2 different QI activities.
    • As you review your records, you can use the same patient charts to answer the questions for both QI projects.
    • This approach allows for consistency in data collection and reflection, making it easier to identify patterns and areas for improvement.

There are several alternative options for sourcing data that CPSA has listed on their website that can provide valuable insights for identifying practice improvement opportunities. These data sources include HQCA panel reports, MD Snapshot Prescribing and EMR data.

You can also leverage data from your groups or systems project to inform the practice-driven QI activity. Reach out to your SCN (Strategic Clinical Network), PCN (Primary Care Network), or physician group to find out more.

Choose Your Data Source for Practice-driven QI

You need to first make a decision about your data source for the Practice-driven QI activity. Factor into account the time, resources and access when making a decision. The following 2 options are available for you in the SUPPORT for PPIP program.

This involves choosing a CPSA Standards of Practice Questionnaire that is relevant your practice.

You have the choice to complete your Practice-driven QI reporting cycle in the My Practice Improvement (MyPI) tool. MyPI is included in your subscription for SUPPORT for PPIP at no additional charge, and is a great way to challenge your quality improvement skills. This tool includes rich coaching prompts encouraging you to engage in deeper self-reflection and helps you generate an action plan, thus satisfying the Practice Driven QI activity PPIP standards.
