Dear Dr Smiley,
Thank you for seeing Mrs Flashing, who at 52yrs of age is struggling with symptoms of perimenopause . Her menstrual cycle is irregular, with bleeding occuring every 2-4 weeks lasting for up to 8 days at a time. She reports 3-4 days of flooding and clots which leave her exhausted . We have trialled hormonal manipulation and she is taking tranexamic acid as per attached medication list on her heavy bleed days. Unfortunately she was not able to tolerate insertion of a Mirena.
She has had 2 normal pregnancies and all PAP tests are normal. There is no other significant gynecological or surgical history
She cannot tolerate the birth control pill and her CBC shows iron deficiency anaemia( copy Attached) A pelvic ultrasound shows large fibroids ( copy attached)
Medical History : nil significant
Allergies : none
FH; Both her mother and sister had total hysterectomies aged 48 and 42 respectively for menorrhagia and fibroids.
I think she would also benefit from a hysterectomy .
